Perineal repair of rectal prolapse involves a surgical approach around the anus and perineum.

One of the most common perineal repair procedures is the Altemeier procedure. During the Altemeier procedure, also referred to a proctosigmoidectomy, the prolapsed portion of the rectum is resected and the remaining ends are reattached. The weakened structures supporting the rectum may be stitched into their anatomical position.

The operation takes around 60 minutes to complete.
